Initial Appointments

You must have a referral from a GP, specialist or nurse practitioner to attend this practice.

Your referral will be triaged by one our Specialists. Our staff will contact you to schedule an appointment.  For non-urgent referrals, please allow up to 3 weeks for this process.

Before your appointment

Please complete the Patient Registration & Privacy Form online by clicking the button below or on arrival at the clinic. If completing at the clinic, please arrive 15 minutes before your appointment time.

What else should you bring to your appointment?

Relevant health cards:

  • Medicare card
  • Veterans Affairs card
  • Private Health card
  • Pension Card

The referral from your doctor or specialist (required at the initial consultation unless the referral has already been sent to our rooms).

Any relevant ultrasound films or reports.

Antenatal patients will need to bring their Antenatal Share Care Card if they have one.

Appointment Cancellation

We require a minimum of 24 hours notice for appointment cancellation. Patients who fail to give appropriate notice will incur a non-attendance fee. Our policy is that no further appointments will be scheduled until this fee has been paid in full.

Payment

Patients are required to pay for their consultation in full on the day of their appointment. Cash, EFTPOS and major credit cards are accepted. We do not Bulk Bill. We can lodge your Medicare claim electronically.
